Our responsibility is to support each client, helping to facilitate services to make their dream a reality. Establishing a support network that meets the client’s care needs and safety, guided by the client’s preferences, is of upmost importance.
A comprehensive, person-centered assessment of your loved one’s current environment, resources, and supports is vital to our services. Whenever possible, meeting in the current living environment is best in order to understand first-hand the needs of your loved one. Upon completion of the assessment, recommendations will be made for services and supports that can “close the gap” of your loved one’s unmet needs. These services can include everything from in-home caregivers to suggestions for quality living communities if needed, and everything in-between.
We will be active members of your loved one’s care team, monitoring services and ongoing needs by telephone and in person to ensure that they remain healthy, safe, and happy. We will maintain close contact with the service providers involved and with you; ensuring good communication is the rule regarding your loved one's care. We will be on-call and available 24/7, able to respond promptly if an emergency or urgent situation should arise.
Occasionally, due to cognitive issues, an individual needs assistance to make important medical, residential, and other decisions to help ensure their own safety and well-being. A Guardian is someone who is appointed by a Probate or other District Court to perform this important role. If chosen and appointed by the court, we will serve as an individual’s Guardian. Some examples of cognitive conditions which may warrant the need for the services listed above include, but are not limited to: all types of dementia including Alzheimer’s, stroke, mental illnesses such as schizophrenia or bipolar disorder, chronic alcohol or drug dependence and abuse, autism spectrum disorders, developmental disabilities, and intellectual disabilities.
Based on our assessment, we will connect you and your loved one to a wide variety of services in the community with whom we have established good working relationships for many years. We will contact these service agencies and make the connections for you, personally introducing you to the service providers, ensuring the highest level of comfort and support.
Planning for the future is an important part of aging with dignity. We are happy to serve as your Medical Power of Attorney to ensure a decision maker is available in the future should you become unable to make medical decisions for yourself. We will conduct a thorough initial assessment and meet with you a minimum of once a year to ensure we are clear about your future wishes and current medical conditions. With fees specifically designed for this service, you will rest easy knowing you’ll be well cared for when the time comes, while living a healthy and active life.
